Blood flow restriction (BFR) training uses a cuff or tourniquet-like band to partially restrict venous return (while still allowing arterial inflow) during low-load exercise.
What are the benefits of BFR?
Strength and hypertrophy at low loads:
Comparable muscle growth to traditional high-load training (70-85% of 1 rep max) while using only 20-30% of 1 rep max
This is the headline finding across dozens of RCTs — mechanistically driven by metabolite accumulation, cell swelling, and increased fast-twitch fiber recruitment despite the low mechanical load
Post-op and early rehab applications:
Big value after ACL reconstruction, knee arthroplasty, and other surgeries where high-load resistance training is contraindicated early on
Helps mitigate quadriceps atrophy in the first weeks post-op when patients can’t tolerate heavy loading
Useful for older adults or deconditioned patients who can’t safely handle heavy loads but need a hypertrophy/strength stimulus
Joint-friendly loading:
Lower compressive/shear forces on joints and tendons compared to heavy resistance training — appealing for patients with osteoarthritis, tendinopathy, or other conditions where heavy loading is painful or risky
Cardiovascular/aerobic component:
Some evidence for aerobic capacity improvements even at low-intensity walking/cycling intensities when combined with BFR
